Registration for Orphan Sponsorship

This registration process allows an orphan child to be enrolled in the Orphan Sponsorship Program.
Please provide accurate information and the required documents to help us ensure proper care and support for the child.

Guidelines for completing Orphan Sponsorship Case Form

  1. Please complete ALL sections of this form accurately. Note that incomplete forms will NOT be accepted.
  2. Do not fill the form in CAPITALS.
  3. To reduce errors, please complete the form electronically, NOT by hand.
  4. The form MUST be signed by the person who completes it.

PHOTOGRAPH

∞ Please include a full length photograph of the child.
∞ Please include a photo of the child’s family, preferably outside their home.
∞ If the child lives in an orphanage you can send a photo of the child with their friends/or the guardian of the orphanage.
∞ Please make sure that the photograph is of a high quality, in particular, ensure that the photograph is not blurry and the lighting is good.
∞ Ensure that the photograph is taken at home or school, NOT at a studio.

  2. Guardian’s National ID Card (NID)
  3. Father’s/Mother’s Death Certificate
  4. Photographs
